AskSophie, is a health platform owned and operated by CloudMedx, Inc. that enables you to search for health and wellness information as a visitor. It allows Members who register to gather and manage their health information online and to receive more personalized health and wellness information.

Personal Information We Collect
AskSophie’s mission is to help you manage your health information and to provide you with personalized health and wellness information. To provide you with personalized services, we need to collect Personal Information about you.
Information We Collect Directly from You
We collect Personal Information directly when you:
Create an AskSophie account. In order to use AskSophie personalized services, you must create an account. To create a AskSophie account, you must provide Personal Information such as name, email address, and a password. This information will help AskSophie to identify you.
Search for health and wellness information. We collect and save your search terms and link them to your computer to facilitate your future searches. We also link these terms to your personal account.
Build your Personal and Health profile. As an AskSophie Member, you can add information to your account in a number of ways. You may upload important documents that contain Personal Information to your Health profile, such as medical, lab, insurance, legal, and other documents. When you use our Personal Information Retrieval Service and you designate us as the recipient of health records you request from third party providers or when you have us directly link to such provider portals, we will collect Personal Information from them on your behalf. You may also choose to include Personal Information about others in your profile by providing us with names and contact information for emergency contacts and health care providers.
Request Health Records. If you choose to request health records, we will ask you to provide Personal Information as needed to complete a request form that complies with the requirements of the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). Such Personal Information may include a Social Security or medical record number. You may also request us to directly link to a health information portal at a third-party health care provider. In order for us to link to a provider’s health portal, you must provide us with your portal credentials (usually your username and password) to allow us to access your health information at those providers.
Contact Customer Service. If you contact AskSophie with questions and requests, we may collect Personal Information from you in order to assist you.
Like most websites, we automatically receive and store certain types of information whenever you interact with our Website. For example, this Website collects the Internet protocol (IP) address used to connect your computer to the Internet; computer and connection information such as browser type, version, and time zone setting. We may collect this information passively using technologies such as standard server logs, cookies, and clear GIFs (also known as “Web beacons”). We use this information to analyze, administer, operate, and improve the Website. Information that is collected in this way generally does not personally identify the user.
We do not link or associate any information gathered through passive means with Personal Information. We use and disclose information collected by passive means in aggregate form or otherwise in a non-personally identifiable information.
As with other Websites, we use "cookies" to obtain certain types of information when your Web browser accesses our Website. Cookies are unique identifiers that we transfer to your device to enable our systems to recognize and remember your device.
The Help feature on most browsers will tell you how to prevent your browser from accepting new cookies, how to have the browser notify you when you receive a new cookie, or how to disable cookies altogether.
When you download and use the AskSophie app, the app stores your device information and stores information similar to a cookie. Due to technical reasons, you are not able to prevent an app from collecting this information. Therefore, if you do not want this information collected, please do not use the AskSophie app.

We use information we collect, including Personal Information, to deliver and improve the AskSophie Services.
Personal Information. We may use Personal Information, to:
Communicate with you. We may send email to the email address you provide us to verify your account and for informational and operational purposes, such as account management, customer service, system maintenance, or to provide information we think may be of interest to you. We may also communicate with you through the AskSophie Services or other means enabled by the AskSophie Services, such as through text messages, push notifications, or in-app messaging. You may adjust your communications preferences in your AskSophie account settings at any time; by unsubscribing from our marketing emails using the link provided at the bottom of those emails; and by sending the appropriate response to stop texts as indicated in those text messages.
Customize your AskSophie experience. We use information we collect through the AskSophie Services to customize your AskSophie experience.
Organize and analyze your health information. When you share information or we receive it from others, our employees and service providers may review that information to categorize and organize it for you.
Provide customer service and improve the AskSophie Services. We use information, including Personal Information, to provide customer service and for internal and service-related purposes.
Aggregate or De-Identified Information. We may aggregate (combine) or de-identify information collected through the AskSophie Services so that you are not readily identifiable. We may use this aggregate or de-identified information for any purpose.
How We May Share Information
We may share information, including Personal Information in the following ways:
With your permission. We may share your Personal Information or other information about you with third parties when we have your permission or at your direction, such as when you direct us to send a request to a health care provider to obtain your medical records or Personal Information.
With our vendors and service providers. We may share any information we receive with vendors and service providers retained to help us provide or improve the AskSophie Services. These vendors or service providers may only use your information to provide or improve the AskSophie Services or their Services.
As required by law and similar disclosures. We may access, preserve, and disclose your Personal Information, other account information, and content if we believe doing so is required or appropriate to: comply with law enforcement requests and legal process, such as a court order or subpoena; defend against legal claims; respond to your requests; protect the rights, property, and safety of you, AskSophie, or others; or as otherwise required by law.
In connection with a merger, sale, or other asset transfer. If we are involved in a merger, acquisition, financing, reorganization, or other substantial corporate transaction, or in the unlikely event of bankruptcy, any information we possess, including Personal Information, may be shared, sold, or transferred as part of such a transaction as permitted by law and/or contract. In such cases, we cannot control how other entities may use or disclose such information.
Selling Data. We do not sell or license (rent) Personal Information. We do not share your Personal Information with other people or nonaffiliated companies for their direct marketing purposes, unless we have your permission.
Aggregate or De-identified Information. We may aggregate (combine) or de-identify information collected through the AskSophie Services so that you are not readily identifiable We may share aggregate or de-identified information with third parties for any purpose.
How You Control Your Personal Information
AskSophie offers you a number of ways to control collection and use of your information when you use the AskSophie Services. Your options include:
Retrieving Medical Records. You may designate which, if any, health care providers from whom you would like us to retrieve Personal Information, including medical records. You may specify which records you would like us to retrieve from any particular provider (e.g., specific dates of service or classes of information like medication lists).
Modifying your account information. You can review and delete the information that you have uploaded or that we have collected on your behalf.
Closing your account. If you no longer desire to use the AskSophie Services, you may close your account directly online. To do so, click on “user profile” and then click “delete account.” After you close your account, you will not be able to sign in to our website or access any of your information. However, you can open a new account at any time. If you close your AskSophie account, we have no obligation to retain your information and may delete any or all of your account information without liability. We may retain and use your information as described in “Data Retention” below.
By using the AskSophie Services or providing an email address or mobile phone number to us, you acknowledge that you accept the risk of potential unintentional disclosure and agree that we may communicate with you via email or text regarding security, privacy, and administrative issues relating to your use of the AskSophie Services.
We will retain your Personal Information for as long as your account is active or as needed to provide you with AskSophie Services. We will retain and use your information as necessary to comply with our legal obligations, prevent fraud or abuse, resolve disputes, enforce our agreements, or take other actions permitted by law. Anonymous or aggregated information that does not identify you personally may be retained indefinitely.
AskSophie Services are intended for persons 18 or over. We do not knowingly collect, maintain, or use Personal Information from children under 18 years of age, and no part of the AskSophie Services are designed for or directed to children under the age of 18. We are particularly aware of privacy protections for children under the age of 13 pursuant to rules issued under the Children’s Online Privacy Protection Act (COPPA). If you learn that your child under 13 has provided us with Personal Information without your consent, you may alert us at email@example.com. If we learn that we have collected any Personal Information from children under 13, we will promptly take steps to delete such information and terminate any account created by such children.
AskSophie is located in the United States and is intended only for use by residents of the United States. Privacy laws for residents of other jurisdictions may be different than those in the United States.
